Medicare prescription drug coverage (Part D) has changed, and these updates are important for anyone enrolled in a Medicare Advantage plan with drug coverage or a standalone Part D plan.
Many beneficiaries are surprised at the pharmacy when they’re told they have a deductible. Here’s what you need to know—and the good news that comes with these changes.
Why You May Hear “Deductible” at the Pharmacy
Starting this year, most Medicare plans include a Part D deductible, especially for brand-name medications in Tier 3 or higher.
This means:
You may pay more at the beginning of the year for certain prescriptions
The deductible usually applies to higher-tier or brand-name drugs
Generic medications may still have low or $0 copayments
It’s important to review your plan’s Summary of Benefits to understand which drugs are subject to the deductible and how your plan structures copayments.
The Good News: A $2,100 Annual Cap on Prescription Drug Costs
One of the biggest improvements to Medicare Part D is the new maximum out-of-pocket limit for prescription drugs.
The annual out-of-pocket cap is $2,100
Once you reach this limit, covered Part D medications cost $0 for the rest of the year
This change is especially helpful for individuals taking high-cost or specialty medications

New Option: Monthly Payment Plan for Prescription Drug Costs
Another important update is a new Part D prescription payment plan option.
If paying higher drug costs upfront is difficult, you may be able to:
Spread your prescription drug costs over monthly payments
Avoid paying the full amount at once early in the year
This option is handled directly through your insurance company, and availability and enrollment details can vary by plan.
